Privacy and scope of services

This website does not require an account and does not sell visitor information. Information submitted through the practice consultation form is used to respond to the enquiry and to evaluate whether contracted RN care-management support is a fit.

The consultation form is intended for practice enquiries only. It is not a patient portal, and patient-identifiable information should not be submitted through it.

Where Anu Health and Wellness provides contracted care-management support to a physician practice, any patient information is handled under the terms of the agreement with that practice, including any applicable business associate agreement, and according to the practice's own policies and procedures.

Patient records remain the records of the contracting practice.

Anu Health and Wellness provides contracted RN care-management support to physician practices. Services are provided within the RN's scope of practice and according to the policies, clinical workflows and applicable requirements of the contracting practice.

Anu Health and Wellness is not a home health agency, and does not provide patient-direct medical services, diagnosis, prescribing, or independent clinical decision-making. The contracting physician practice retains responsibility for the patient relationship, clinical decision-making, patient eligibility determinations, and its applicable billing and reimbursement processes.

Nothing on this website should be read as a representation that a practice will receive any particular reimbursement, revenue, or program outcome, or that any given patient is eligible for a particular program. Those determinations rest with the practice and the applicable payer requirements.
